Job Description
Continuum Behavioral Health – a subsidiary of The Providence Center, has launched a new program called HealthPath for commercially insured Blue Cross Blue Shield adult members.  The program offers a collaborative, team-based, recovery-oriented approach to care in a safe, therapeutic environment. The program is designed to treat clients with serious mental illness and help to stabilize clients, implement effective treatment strategies, coordinate care and discharge clients back to their lives with appropriate supports. Clinical services are tailored to the unique needs of each client through comprehensive assessments, care coordination, therapy, education, relapse prevention, recovery management and discharge planning. To ensure the success of this program,  HealthPath is looking for a full-time Clinical Therapist who is experienced in working with clients with serious mental illness.
***please note that the person being hired cannot be a current CNE employee or an employee of any CNE operating unit
Required Education
Master’s degree in Social Work or related field and clinical experience.  LICSW OR LMHC OR LMFT AND LCDP required. DBT certification/training strongly preferred.

Ability to write reports and correspondence. Ability to speak effectively with consumers, community agencies and Center employees.

Ability to apply commonsense understanding to carry out instructions furnished in written or oral form. Ability to deal with problems involving several concrete variables in standardized situations.

Insured auto and valid driver's license.  LICSW strongly preferred.
